Type of inspection: Renewal An unannounced, mandated renewal inspection was held on 04/09/2024 from 12:40 p.m. to 3:45 p.m. A preliminary findings review was held on the same date regarding resident environment and records. Staff records were reviewed virtually on 4/10/24 from 5:00 to 6:00 p.m. The Acknowledgement of Inspection form was provided via e-mail. An exit interview was held on 4/16/24. Number of residents in care: 9 The licensing inspector completed a tour of the physical plant that included the building and grounds of the facility. Number of resident?s records reviewed: 4 Number of staff records reviewed: 5 Number of interviews conducted with residents: observation only Number of interviews conducted with staff: 2 Additional Comments/Discussion: The evidence gathered during the inspection determined no violations with applicable standard(s) or law.
