Comments:

An onsite inspection was conducted on October 20, 2021. This inspection was conducted in response to a new application for licensure and the facility is not yet in operation. The applicant was contacted by telephone to initiate the inspection. The inspector emailed the applicant a list of items required to complete the remote documentation review portion of the inspection. Personnel documentation for two staff and three board officers was reviewed. Facilities policies and procedures and forms were reviewed to ensure documentation was complete. On October 20, 2021, the inspector conducted an exit meeting with the applicant where findings were reviewed and an opportunity was given for questions as well as for providing information. The applicant was advised that any additional documentation needed would be requested by email and no documents were exchanged on the date of the site visit. Background check results for personnel and board officers had not been received as of the date of the on-site inspection. Child Protective Services Central Registry search results, national fingerprint based criminal history search results, and Sworn Disclosures for Personnel 1 through 12 (P1 through P12) and Board Officers 1 through 3 (B1 through B3), and Agent 1 (A1) must be reviewed by the licensing inspector before a decision can be made on the application for licensure. Please provide background checks, using a secure delivery method, for review by the licensing inspector upon receipt. Provide personnel records for Personnel 2 through 11 (P2 through P11) and any additional staff hired. Information gathered during the inspection determined no non-compliance with applicable standards or law, and violations. However, if review of the background checks or personnel files results in non-compliance findings, the findings will be documented and issued to the facility.
